"Sore Tongue"
 
   Within a couple weeks of having all four wisdom teeth surgically removed, I noticed pain in my tongue. When it did not go away, I looked at my tongue in the mirror, and it had a slight white coating. Having never experienced a sore or whitish tongue before, so I thought I might have gotten oral thrush from taking antibiotics after the surgery. (I had never seen thrush and did not know what it would look like.)

It is now nine months after the surgery, and my tongue is still slightly sore with a little bit of numbness, mainly just toward the front of the tongue. I have been treated with antifungal medications twice, and antibiotics once, and nothing helped. Tonight I just stumbled upon this web site, and I now feel with absolutely no doubt that I have nerve damage from some aspect of the wisdom teeth removal.

I spent several months going to my doctor and being treated for conditions which I don't even have. I never knew that nerve damage was even a possiblity. It feels really good to finally know what is wrong, and I feel lucky that my pain is not severe, just a minor irritation. The only thing that puzzles me is the slight white coating on the tongue. Has anyone else had this? I did read in one medical book that a white coating can be caused by injury to the tongue.

Now, close to a year after the surgery, is there any reason to contact the oral surgeon who took my teeth out? My case doesn't seem very sever compared to most others here, and I didn't even tell him that my tongue was damaged, because I did not know what was wrong until now. I would appreciate the thoughts of anyone who has experienced something similar.

   Angie,

I was so thankful to read your post. My lingual nerves were injured from my wisdom teeth extractions, but they have mostly recovered. My tongue gets slightly irritated, especially with stress. My tongue too has had a slight white coating in some areas since the injury, so I bought a tongue scraper and diligently clean my tongue morning and night so that it is sparkling clean but it still gets the slight whitish in areas.

After reading your post it hit me--maybe the decreased innervation in areas means the cells aren't being replaced and renewed as efficiently as before, so hence the slight whitish cast. I'm not sure if tongue cells are renewed like skin cells, but maybe that's it.

I find that exercise and good energy really really help.

   Ursula, thank you for your response! I had eventually started brushing my tongue well, morning and night, and that did help the whiteness, just not the soreness. I have always taken good care of myself, so I felt sure that I didn't have a nutritional deficiency or something like that causing my problems. But of course now that I realize that I have nerve damage, it is all explained.

Good luck with a full tongue recovery -- you have helped my state of mind! It is nice to know that what I have been experiencing is not completely out of the ordinary.

   The whitish color to the tongue sounds interesting. The tongue epithelium is a squamous mucosa, with a lot of interesting aspects to it because of the specialized innervation for taste. Like the skin, it does constantly regenerate cells. It can be effected by some disorders that effect the skin, some of which can cause whitish patches. The relationship of the whitish patches to nerve damage is not immediately apparent to me, so I will have to look around.

It would be interesting to have an opinion from a dermatologist as well as an ENT physician.

Do the white patches only occur on the side of the nerve damage? Does it effect the same region as the nerve damage, or are they small patches? Do they come and go, or is it pretty constant? Does the tongue injury cause you to rub the tongue on teeth, etc?

Let me think about this one and ask around.

   The whiteness is not in patches -- it covers the entire tongue, but is whiter in the back than in the front (my tongue is only sore on about the front inch of the tongue). The tip of the tongue and the outer edges of the tongue always stay a normal pink color. The whiteness is very similar to what you might sometimes have very mildly first thing in the morning, especially if you have a slight cold or something. It is not really much of a covering -- the texture of the tongue is the same as a normal tongue, and it is not in plaque-like spots (like with thrush.) If I brush my tongue every day it stays a normal pink color. The whitish color is constant unless I brush the tongue once or twice a day, which I do now to keep it pink. My American Medical Association Family Medical Guide says that glossitis, an inflamation of the tongue (which can be white), is sometimes caused by tongue injury.

My doctor has treated it with two different anti-fungal medications and a round of antibiotics, none of which affected it at all. The pain I feel in my tongue is similar to what you feel after burning your tongue on a very hot drink -- a little sore yet a little numb, sometimes a little tingly. It started feeling this way immediately after having my wisdom teeth removed. It is not as sore as it was right after the surgery, but it did not improve much after the first month or so.

The tongue injury does not cause my tongue to rub on my teeth. Besides, the whiter part of the tongue is back where the teeth can't touch it anyway. I'm not too surprized that you're not familiar with it -- my doctor has been completely stumped. Extensive blood tests have ruled out any underlying systemic cause of the whiteness. I have (and have never had) any type of dermological problems.

   I too have had a white coating on my tongue since my nerve injury from a wisdom tooth extraction three years ago. When I went back to the oral surgeon's office shortly after the extraction, her assistant made a remark to me about it (in a nasty way.) Nothing seemed to help except daily brushing. Now I never brush my teeth without brushing my tongue.

   I also have a white coating on my tongue. It changes--sometimes its worse than others. Sometimes, it almost seems like its flaking and looks "dry" and "grainy". I noticed it right after my wisdom tooth was removed. I mentioned it to my oral surgeon but he never address it and I had larger issues to deal with. I figured it would go away, but it never did. I'm surprised no one has ever mentioned this before. I never thought to because I wasn't sure what it was from.

My white coating is also "thicker" in the back of the tongue and covers the whole tongue, not just the damaged side. I also notice that my mouth gets drier easier than it used to.

I bought a tongue scraper but it doesn't always seem to work. some days the coating is thicker than others.

Has anyone else tried anything to get a pinker tongue? I am curious to know the reason for this and how many people have experienced this.

   I have had a white coating on my tongue and the color of my tongue seemed off. Recently I bought Bach's Rescue Remedy which helps soothe and heal the skin. It is made from flower essences. It has helped more than anything. The flavor causes me no discomfort at all which is unusual for me at this point in the recovery. The normal color of my tongue is beginning to reappear. I think it has helped reduce the swelling of my tongue because the teeth marks appear lessened.

   It is such a relief to know that I am not the only one and others have had similar experiences. Your story could have been written by me.

I too had a wisdom tooth extraction upper left with everything appearing normal for two to three weeks then suddently I'd get a very dry mouth, as if someone had stuffed a load of cotten wool in my mouth and I instantly had to get a drink. This would last for 5 minutes or so and subside.

Then I had some inflammation in the side of my mouth which the doctor prescribed penicillin for. However I felt very sick - strange as I don't normally react badly to antibiotics. It was all downhill from there. I ended up in casualty because I could get no relief from the awful sensation in my mouth and the dryness which left my tongue very white. The doctor at casualty said I ought to see an ENT specialist just in case it a saliver problem, as one needs the enzymes to digest food and keep the mouth in good order. My doctor disputed this and said I need an oral surgeon. Unfortunately my tongue problem wasn't considered a problem and I received a biopsy for the inflammation on the cheek which was negative.

I was off work for three months, because I was tired, could only eat a very bland diet, mainly fish bananas and mash, anything that didn't irritate my mouth. In desperation I asked the doctor for nystatin for thrush and my tongue reverted to something resembling my tongue. I've had two more courses of antibiotics which resulted in a white tongue again, followed by two more courses of Nystatin.

My tongue hasn't been itself for some time. Like yours it goes white all over except for the sides and tip, especially if I'm subject to strong odours, perfumes, hosehold chemicals.

The confusion lies in whether it was wisdom extraction induced or thrush induced.

I carry a synthetic oral saliver spray and hope that the problem sorts itself.

I'm having an MRI scan soon to investigate the burning and pain in my top and bottom left side. The pain in the site of the extraction was explained by the dentist as being the bone healing which could take a few months.

I'm also on a yeast free diet and have read a book on candida which on reflection details some of my symptoms

Conclusion: Thrush low grade brought on by infection in the mouth after extraction and/or antibiotics,

OR

alteration in mucosa and sensation in the mouth due to damage after extraction.

OR BOTH

I think I could deal with the problem more if Doctors were less than baffled unless of course they feel liable in some way. They dismis the symptoms as neurotic etc......

   i am also experiencing the tounge pain and pain in my mouth, the top of my mouth feels blistered, my tounge burns and hurts the sides of my teeth will feel like i have a tooth ache in all my teeth. i too have taken anti-fungal meds. and my problems went away for a year. i took steroid injections in my shoulder and wrist and my symptoms have returned. the only thing that concerns me is that i have horrible headaches with my outbreaks. and i too am very limited to what i can eat. i use hydrogen-proxide that seems to help. i have a rx for liquid lidocane that also helps as well as oragel. i also have a burning sensation in my nose that i havent read anyone else talk about. ice on your mouth will help and sometimes i get relief from a heating pad and a massager. i place the massager on my teeth and the pain will go away. also if you will chew gum it will help with the tounge pain. your tounge can not hurt and tase at the same time. this is a night mare and i hope the steroid will get out of my system and my symptoms will go away. i think i have candida and nerve damage in my mouth. and your correct doctors will not even talk to you. they think your crazy. good luck, i hope we all recover soon.
