"Mulitple issues after wisdom tooth extraction"
 
   I feel like I am living a nightmare all the issues caused by the extraction. I am in a great deal of pain and can't sleep. Please give advice. Should I see a 3rd dentist, should I go right to an oral surgeon, should I go to my doctor, should I wait it out? Help!

Here is the story: I had one wisdom tooth extracted (lower-right) 2.5 weeks ago. While getting the injection I felt a shot of tingles through the right side of my tongue. I still do not have feeling on the right side of my tongue. Both dentists (yes plural) told me I would get the feeling back, but not for weeks. Plus, the immense pressure the dentist put on my lower jaw while trying to use "leverage" to get my tooth out has caused so much pain (I have TMJ too) that I cannot open my mouth more than a half inch making it difficult to eat, and he stretched the size of the socket so much that is now a gaping hole in my gums.

This is compounded by dry socket and the fact that the first dentist would not diagnose my dry socket. And now the second dentist, despite the fact that he diagnosed the issue, has told me there is nothting he can do for me because the packing keeps falling out (because the first dentist butchered my gums and made the socket huge while man-handling the wisdom tooth).

It is painful to eat, breath, sneeze, yawn, and I cannot sleep. Help?

   Alot of us here know your nightmare well .. Im glad i didnt take the advice of the dentist who caused my lingual nerve damage 7 years ago... Sounds to me like your 2 dentists are in denial about your injury just like mine was .. I cant even believe 1 out the 2 have not referred you to an OS YET ? SHAME on them !! Yes you need to SEE one.. Preferably an OMFS is who might be able to help you .. But to get medicated for pain and not being able to sleep you need to see your GP for that ..

   bowho - Thank you, for "feeling my pain" so-to-speak. Shame on them is right. I felt in my gut something wasn't right, but until I found this site I had nothing to go on. I feel empowered now.

However, you'll have to forgive me, but I have no idea what an OMFS is. Could you spell it out?

   I have felt your pain well.. I also have a right side numb tongue.. I still have problems eating and sneezing ... I can breath better since ive dealt with this for what seems like forever now .. If you think the dentists are negligent in your aftercare you might want to talk to a lawyer.. And let the dentists know your not going to put up with their ignorance one more minute... I see DR B clued you in on OMFS ...

   I will definately go see an OMFS. After all I have been through, I want to make sure I see a highly regarded OMFS. Are there any resources I could access to find the best OMFS?

   That is quite a hike, as I live in SE Wisconsin. However, that may be my only option.

I've called two OMFS already and one said they cannot help - recommended I see a micro neurosurgeon. The other said - she would prefer I continue working with my general dentist (???). Is there some code among dentists that no one will give me a second opinion?

   There is a code among most medical doctors/dentists. It is difficult to find help after one of their peers screwed up on you. Even worse is trying to get the regulatory board (dental board in your case) to put any stock in what you report. You should report this situation to your state dental board.

Keep looking and if nothing else, don't embellish on the reason that you want to come. You could also try getting a referral from your primary care physician.

I actually went to see a microneurosurgeon in Milwaukee. He perscribed steroids and suggested I go see a Ear Nose & Throat specialist.

He said even if my lingual nerve was damaged it wouldn't be "worth" a nerve graft (?) because there is no loss of mobility. He also thought the feeling would probably come back and I should just wait.

Does that even happen?

   Me too! I actually left Steinberg a message on Wednesday; however, I have not heard back. I will try him again.

Since I do not seem to be speaking to any reputable or knowledgable MD's lately. Would you mind reassuring me of Dr. Steinberg's credentials? What are the reasons you are recommending him? How do you know (or know of) Dr. Steinberg? I would really appreciate any information you can provide.

Thank you so much for all of your help. I will keep you updated on my status.

   Oh for f$#@ sake !!! I really hope you didnt pay this asshole any money for that ignorant advice ? You should really find his/her email and show them this forum ... I got feeling back in the tip and back of my tongue only about 14 to 18 months after the damage occurred and that was only after nerve repair surgery took place.. I always had mobility... Mobility has nothing to do with the numbness... There really is no other thing than TIME to see if you get your feeling back but if it doesn't start happening in the first few months chances are it wont.. Are you in pain also ? I had very painful numbness.. Im in Cleveland Ohio and so is the OMFS who helped me ...

   You sound as frustrated as me. The neurosurgeon admitted he did not know anything about the lingual nerve. He spent a great deal of the appointment basically giving me a physical (hammer on the knee to see me kick kind of things).

I made a point of explaining to him that it was really *my* decision if surgery was worth it and since I do not want to deal with this excruciating pain, numbness (with freezer burn sensation when ibuprofen wears off), and excess amounts of ibuprofen (hate Rx pain killers) forever... I think it would be worth it. He wasn't too happy with me not just smiling and nodding my head.

The problem is... other than driving all the way to Illinois, I don't have any recourse here. I'd like to just wait it out becuase it has only been three weeks, but I don't know how long is too long (or too late).

   Actually, Boo Hoo is quite correct. Mobility has nothing to do with sensation loss and this guy sounds like an idiot. I practiced in Chicago for 25+ years and Steinberg was the guy to go to for such injury. He is an oral surgeon with much additional training. You are best seen in the first 2 month period of time.

Thank you both so much for your help and support. I actually recieved a call from Steinberg this evening. He gave me the name of an oral surgeon with similar expertise at Froedtert Hospital here in Milwaukee. I feel so relieved already knowing that I will have the chance to speak to a knowledgable doctor.

I think I might have gone a bit crazy in these past few weeks without your expertise and empathy. I've only broke down in tears once.

I will let you know how it goes!

Glad to hear you got hooked up with a Dr who might have experience with oral nerve damage.. Not a whole lot of people get hooked with the right kind of medical attention and suffer for a long time because they are simply told to wait and see what happens.. Of course it really is a waiting game and that's a fact of the injury.. Nerves don't heal overnight..
I felt the electrical shock of the lidocaine injection also.. I also felt the shock of the wisdom tooth extraction..
I never had the metallic taste.. Some people do.. i simply had no taste and no feeling in my burning, yanked out of place tongue.. with a vice grip feeling on the jaw... My tongue snapped back in place one day after a neuroma was removed .. I regained feeling in the tip and back of my tongue.. I only regained taste on the back and the middle remains numb including my gums.. I found that taste and feeling are not the same injured nerve..

Yes, I went back and read my post. I must have been distracted. I have no idea why I typed "hormone" when I meant "steroid."

The pain in that socket has subsided since I started the steroids. I have not taken any ibuprofen since Sunday. YEAH! It is still sensitive to hot and cold, but much much better than it was before.

Dr. Steinberg sounded truly interested in helping me get the best care... I cannot thank you enough! I have an appointment in early February with OMFS Dr. Steven Sewall at Froedtert & The Medical College of Wisconsin. I will let you know my experience and if Dr. Sewall is "referrable."
